azTeen General Contest RulesNO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT 1. How To Enter An azTeen Contest. A. On-Line Entry: Unless otherwise directed, to enter an azTeen Magazine or azteenmagazine.com contest online, visit the respective contest web page at www.azteenmagazine.com and complete and submit an online entry form during the specified entry dates. Only one Internet entry per person and one Internet entry per email address are permitted. Internet entries will be deemed made by the authorized account holder of the email address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the email address by an Internet access provider, online serve provider or other organization that is responsible by assigning email address or the domain associated with the submitted email address. azTeen Magazine and azteenmagazine.com are not responsible for entries not received due to difficulty accessing the Internet, service outage or delays, computer difficulties or other technological glitches. B. Participants assume all risks of damage, lost, late, misdirected, postage-due, incomplete or illegible entries. Facsimile and mechanically reproduced entries will not be accepted. All entries submitted become property of azTeen Magazine, azteenmagazine.com and its affiliates and will not be returned. Multiple entries from the same person will be eliminated from the eligible entries. 2. Elgilbility Restrictions: A. Unless otherwise noted, azTeen Magazine or azteenmagazine.com contests are open to all legal residents over the age of 13 years and under the age of 20. azTeen Magazine or azteenmagazine.com contests are void where prohibited by law. All federal, state, and local laws and regulations apply. B. azTeen Subscribers and azTeen VIP members are eligible to win a prize in an azTeen Magazine or azteenmagazine.com contest. Only one winner per household is permitted in any contest. Participants that win an azTeen Magazine or azteenmagazine.com contest prize valued at $600 or more will be required to complete a release form. C. For online contests, entrants are required to provide truthful information and azTeen will reject and delete any entry that it discovers to be false or fraudulent. azTeen will also disqualify and delete any entry received from persons under the age of 13. 3. Prizes. A. Offered prizes and their approximate retail values will vary as noted. Unless otherwise noted, all prizes associated with azTeen contests, may be picked up at the offices of azTeen Magazine or its affiliates. The winner will forfeit any prize or prizes not claimed within thirty days of winning. In the event that a prize or prize certificate is mailed to winner, it will be with the prior written or oral consent of the winner and therefore, winner assumes the risk or its loss. azteen Magazine is not responsible for the safe arrival of a prize or prize certificate. B. There is no substitution, transfer, or cash equivalent for prizes, except that a Sponsor may, at its sole discretion, substitute prizes of comparable value or cash. The prizes are expressly limited to the item(s) listed above and do not include taxes, gratuities or any other expenses. Other restrictions may apply. 4. Selection of Winners: A. Decisions of azTeen Magazine or azteenmagazine.com with respect to the contest are final. B. Unless otherwise noted, potential winner(s) are selected in random drawing(s) of all eligible entries received during the entry period(s) by the staff of azTeen Magazine or its contest sponsors. C. Odds of winning depend on the number of eligible entries received. D. Potential winner(s) may be required to execute and return any required Affidavit of Eligibility and/or Release of Liability Prize Acceptance Form within ten (10) days of notification attempt or prize will be forfeited. 5. Conditions: A. Payment of all federal, state, and local taxes, are solely the responsibility of the winner. Winner may be required to sign an IRS form W-9 or the equivalent. B. By participating in the contest, the winner or winners agree to have their name, voice, or likeness used in any advertising or broadcasting material relating to this contest without additional financial or other compensation, and, where legal, to sign a publicity release confirming such consent prior to acceptance of the prize. C. Prior to awarding any prize or prize certificate, azTeen Magazine in its sole discretion may require contest winners to sign a liability release, agreeing to hold azTeen Magazine, its owners, corporate licensee, its sponsors, subsidiary and affiliated corporations, and the officers, shareholders, directors, employees, agents and representatives of each of them harmless against any and all claims or liability arising directly or indirectly from the prize or participation in the contest. D. azTeen Magazine, in is sole discretion, reserves the right to disqualify any person tampering with the entry process, the operation of the azTeen Magazine Web Site or is otherwise in violation of the rules. It further reserves the right to cancel, terminate or modify the contest if it is not capable of completion as planned, including infection by computer virus, bugs, tampering, unauthorized intervention or technical failures of any sort. E. azTeen Magazine reserves the right to make any changes in the rules of the contest, including the substitution of a prize or prizes of equivalent or greater value, which become effective upon announcement. F. Failure to comply with the contest rules may result in an entrant’s disqualification, at the sole discretion of azTeen Magazine. G. azTeen Magazine is not responsible for typographical or other errors in the printing, the offering, or the administration of the contest or in announcement of a prize. H. Copies of the written contest rules and a list of all winners are available during regular business hours at offices of azTeen Magazine, 3219 East Camelback Road, Suite 363, Phoenix, AZ 85018, on www.azteenmagazine.com, or by sending a self-addressed stamped envelope to azTeen Magazine at the same address. |
